J 1 visa - sponsor found and now?

My daughter Francine has found an sponsor for the J 1 internship visa. We know a little bit, but the agrement with the company is that she does the paperwork. So here are some questions and we hope you can help us. That would be so awesome.

1. What form has she to fill out?
there are two kinds. with one you have to go back after the visa ended and you're not allowed to come back to the US for 2 years, we don't wnat this )

2. Where does she find the right forms?

3. How long will it take to get the visa, in general?

4. What about insurance for the intern?

5. How much does it coast to do this visa?
